Some services provided are not covered under our contract with the NHS and therefore attract private fees. Examples include the following:

  • Insurance claim forms
  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

For any other private item of service, please contact the practice and ask our Reception Manager, Medical Secretaries or Practice Manager.

 

Firearms - Medical Certificate  

From 1st September 2020, all applications (and renewals) will NOT be processed by the firearms licensing department unless they contain a medical report, which will be limited to fact rather than an opinion. This is a change from the previous system whereby the application was processed after 21 days, even without medical information, on the assumption that no medical information means the Doctor had no concerns.

A new proforma/guidance form for applicants to submit to their GP or another doctor must now be completed. This proforma makes it clear that the completion of this information by the Doctor is not commissioned through an NHS contract and as such the applicant can expect to pay a fee for this service, and that this may be required upfront. 

If you wish to request the completion of this application by your Doctor, please request a form from reception, complete applicant details and tick whether is is a new or renewal application, then bring/send the form to the Partnership addressed to the Medical Secretary with a covering note. Please allow 30 days for completion, which will not commence before a fee is paid.
